The Care Package

The care package will be designed specifically for your needs or your client’s needs. Our clinical team will work in close partnership with the relevant care manager / case manager and other professions, to ensure the carers provided are trained and competent in the skills required to care for you or your client.

It is as important to us to provide you or your client with an enjoyable social life, as it is to provide excellent standards of nursing and personal care. Hobbies, outings and study will be actively encouraged.

If required, carers will work under the supervision of therapists to ensure therapy treatment programmes are followed and goals reached.

Our aim is to ensure care packages are focused on maintaining optimum independence and quality of life for you or your client.

An enabler and nursing care service provided by fully trained and competent carers under the supervision of specialist nursing staff.

Referral

Referral will be taken by Partners in Specialist Care via the following routes:

• Directly by client or their family
• Primary Care Trusts
• Local Authority Social Services Departments
• Case Manager or Advocate
• Legal Representatives
• Insurance Companies

Funding for the care package must be agreed before recruitment to the package commences.

Costs can be provided following a full assessment of your needs or your client’s needs.

Partners In Specialist Care Limited is registered with the Commission for Social Care Inspection to provide Domiciliary Care Services to adults and children.

The company is accredited by a number of Local Authorities to provide home care services.
